Every time you finish a topic, it is evaluated - you get graded on your performance. Finish enough topics, get enough points and you rank up. There is no formality. Ranking Up requires two things however - EXP and MP. MP is the grade/points you get for missions, whereas EXP is the grade/points you get for all the other topics. Because of this, the MP requirement is smaller than the EXP requirement.
