That depends on whichever village you are starting from. If it is Kumogakure you need to speak with .Tora if it is Konohagakure you need to speak with Captain Konoha. Just PM them and ask for their permission.

If you are applying for the Uchiha, you have to start as a genin.

If not, as Kiseki said, you must asked the one of the kages for permission.
